According to the NYC Department of Health, all participants must have a completed medical form (must be one year from the date of issue) prior to the start of the program. It’s also a good idea to keep extra copies of the medical form for your records. If your child does not have a complete medical form along with the health care practitioner's stamp or signature, they may not attend.
